Open wound in scrotum

Hi, my cat have an open wound in scrotum. I bring him to the vet twice already, but it still not healing. Today is the second day after finishing his second round of antibiotic.

The problem is he keep licking them. Even with the e-collar on. It seems like he will try anything to get to it. Sometimes even make an angry cry.

I ask for a bigger e-collar but the vet said he wouldn't be able to eat or drink if the collar is bigger. I am putting some antibiotic spray with bitter taste too... It didn't stop him from licking...

The wound is red and swelling. It started happen after one month I neuter him...
I did ask the vet to stitches it back but the vet said scrotum skin cannot be stitches if its still swelling...

I have no idea what more to do but to monitor him as much as i could.
He is so persistent. i hope you all kind hearted people have some clues on how to deal with this issue.

Gracecy87, I strongly suggest you take your cat to another vet for a second opinion!
